Support and rehabilitation after hospital stay helps client to make full recovery. Where needed, we can continue treatment at home avoiding unnecessary journeys back to hospital.
Our Live in Carers can support people with chronic illnesses and fit around your personal routine, perhaps providing medication at home. For short trips, perhaps a doctors’ visit, a New Pastures Live in Carers companion can be on hand to help.
Each package is tailored to the needs of each individual client which can range from daily 24 hour Care to Live In Care through to around-the-clock support or round the year support.

Live-In Care

Live In Carers Duties include:
Alzheimer’s’ and Dementia care
Making appointments with General Practitioners and all health professionals
Bathing and personal hygiene, dressing, grooming and appearance
Changing and washing bedding
Wake up and Assisting with Nighttime routine.
Light housekeeping, laundry, meal preparation and planning
Companionship and conversation

Grocery shopping and errands
Medication prompting and reminding
Motion exercises e.g. walking and mobility assistance, reading to care recipient
Reporting the day to day to the family
Bill payment and writing

Independent Living Support

New Pastures Live In Carers has trained health care assistants, home carers, cleaners etc. that support our clients with minor to long-term health issues who prefer to live independently.
We have found out that it can often delay the need for long term nursing home care whilst enjoying privacy and 100% care for everyone. We promote independence, enabling each individual to do what they desire and are capable of.
New Pastures Live In Carers monitors changing needs and expectations to enable our clients to enjoy life as well as get the most out of life. The clients we work with have a variety of complex support needs such as physical disabilities, dementia, mental health, learning disabilities, spinal injury, acquired brain injury, etc.

Night Care
A dedicated Night Care service provides supervision and support throughout the night, ensuring service users feel safe and supported while reducing risk of falls, distress or unmet care needs.
Day Sitting (12-Hour Service)
Day Sitting provides structured daytime support, ensuring service users are assisted, engaged and supervised while families attend to work or other commitments.
Companionship and Social Support
Recognising the importance of social connection, Companionship Services support emotional well-being through conversation, shared activities and accompaniment to appointments, errands and community outings.
